A note about Residential Services--

Residential Services come in many forms and adaptations, but planning for such services begins with the DD Program of the Mid-Columbia Center for Living. The DD Program's Service Coordinators work with the individual, their family and other resources to help develop the residential plans that the individual wants.

•An adult living on one's own in an apartment or house with In-home support or "personal services" tailored to the individual's needs.

• An adult or child living in the family home with in-home supports or personal services necessary for the individual to continue to live in the family home.

Depending on the individual circumstances and funding availability, funding for in-home supports may come through the Eastern Oregon Support Services Brokerage, the Mid-Columbia Center For Living's programs such as Comprehensive Services, or Family Support Program.

Where the individual is enrolled in the Eastern Oregon Support Services Brokerage, funding for in-home supports as outlined in the individual's person-centered plan is made available through the brokerage.

Service providers include individual service providers hired by the consumer or family, or staff of the Columbia Gorge Center's Supported Living or Semi-independent Living programs. Referral and funding for residential services comes through the Mid-Columbia Center for Living for Supported Living Programs, 24-hour group homes and foster homes.

• Group Home Living in 24-hour staffed homes of 3-5 residents. The Columbia Gorge Center, Inc. and Continuous Consumer Improvement, Inc.are two providers of 24-hour residential services in the Mid-Columbia.

• Foster Homes licensed for children or adults.
